Expansion of the Adherence Club model for stable antiretroviral therapy patients in the Cape Metro, South Africa 2011-2015

ObjectiveThe ambitious 90-90-90' treatment targets require innovative models of care to support quality antiretroviral therapy (ART) delivery. While evidence for differentiated models of ART delivery is growing, there are few data on the feasibility of scale-up. We describe the implementation of the Adherence Club (AC) model across the Cape Metro health district in Cape Town, South Africa, between January 2011 and March 2015. MethodsUsing data from monthly aggregate AC monitoring reports and electronic monitoring systems for the district cohort, we report on the number of facilities offering ACs and the number of patients receiving ART care in the AC model. ResultsBetween January 2011 and March 2015, the AC programme expanded to reach 32425 patients in 1308 ACs at 55 facilities. The proportion of the total ART cohort retained in an AC increased from 7.3% at the end of 2011 to 25.2% by March 2015. The number of facilities offering ACs also increased and by the end of the study period, 92.3% of patients were receivingART at a facility that offered ACs. During this time, the overall ART cohort doubled from 66616 to 128697 patients. The implementation of the AC programme offset this increase by51%. ConclusionsACs now provide ART care to more than 30000 patients. Further expansion of the model will require additional resources and support. More research is necessary to determine the outcomes and quality of care provided in ACs and other differentiated models of ART delivery, especially when implemented at scale. Bien que les donnees sur les differents modeles de prestation de l'ART soient en croissance, il existe peu de donnees sur la faisabilite de leur deploiement. Nous decrivons ici la mise en OEuvre du modele d'adhesion au club (AC) dans le district de sante du Cap Metro, a Cape Town, en Afrique du Sud, entre janvier 2011 et mars 2015. MethodesEn utilisant les donnees de rapports de suivi mensuels d'AC cumulees et des systemes de surveillance electronique pour la cohorte du district, nous rapportons sur le nombre d'etablissements offrant l'AC et le nombre de patients recevant des soins ART dans le modele AC. ResultatsEntre janvier 2011 et mars 2015, le programme AC s'est elargi pour atteindre 32.425 patients dans 1.308 AC dans 55 sites. La proportion de la cohorte totale sous ART maintenue dans une AC est passee de 7,3% a la fin de 2011 a 25,2% en mars 2015. Le nombre d'etablissements offrant l'AC a egalement augmente et a la fin de la periode d'etude, 92,3% des patients recevaient l'ART dans un etablissement qui offrait l'AC. Pendant ce temps, la cohorte globale sous ART a double, passant de 66.616 a 128.697 patients. La mise en OEuvre du programme AC a contribue a 51% de cette augmentation. ConclusionsLes AC offrent maintenant les soins ART a plus de 30.000 patients. L'expansion plus elargie du modele aura besoin de ressources et de soutien supplementaires. Des recherches supplementaires sont necessaires pour determiner les resultats et la qualite des soins fournis dans les AC et d'autres modeles differencies de prestation de l'ART, en particulier lorsque implementes a l'echelle. Aqui describimos la implementacion del modelo del Club de Adherencia (CA) a lo largo del distrito sanitario de Cape Metro en Ciudad del Cabo, Sudafrica, entre Enero 2011 y Marzo 2015. MetodosUtilizando datos de los informes mensuales agregados de monitorizacion de CA y de sistemas de monitorizacion electronica para la cohorte del distrito, reportamos el numero de centros que ofrecen CAs y el numero de pacientes que reciben TAR dentro del modelo CA. ResultadosEntre enero 2011 y Marzo 2015, el programa de CA se expandio hasta llegar a 32,425 pacientes en 1,308 CAs de 55 centros. La proporcion de la cohorte total retenida en CA aumento del 7.3% al final del 2011 hasta el 25.2% en Marzo 2015. El numero de centros que ofrecian CAs tambien aumento hacia el final del periodo de estudio, 92.3% de los pacientes estaban recibiendo TAR en un centro que ofrecia CAs. Durante este tiempo, la cohorte total de TAR se doblo de 66,616 a 128,697 pacientes. La implementacion del programa de CA compenso este aumento en un 51%. ConclusionesLos CAs ahora proveen cuidados de TAR a mas de 30,000 pacientes. Una mayor expansion del modelo requiere recursos y apoyo adicionales. Se requiere mas investigacion para determinar los resultados y la calidad de la atencion ofrecida en CAs y en otros modelos diferenciados de entrega de TAR, especialmente cuando se implementan a escala.
