Partial Splenectomy in a Case of Granulosa Cell Tumour (GCT) with Isolated Splenic Metastasis: A Case Report with Review of Literature

A 50-year-old female, a case of granulosa cell tumour of the ovary, operated multiple times, post-chemotherapy and hormonal therapy, since the last 12years was detected to have a splenic parenchymal metastasis on follow-up. Owing to poor results with our experience with total splenectomy on two postoperative cases of ca ovary, we decided to perform partial splenectomy. Intraoperatively, the nodule was localised with intra-op USG, during which two additional nodules were found and excised. Final histopathology showed tumour in two of these nodules. Intra-op USG-guided RFA of the spleen lesions with minimally invasive techniques was another option but since the patient had a concomitant incisional hernia we decided to proceed with open approach along with hernioplasty. In English literature, spleen as the only site of metastasis is exceedingly rare. We report the first case of granulosa cell tumour of the ovary which has been treated with partial splenectomy. Carrying out this case successfully leads us to believe that partial splenectomy is a viable option in cases where spleen is the only site of metastasis, which also protects the patient from possible complications of splenectomy.
