Management of the Recurrent Deformity in a Flexible Foot Following Failure of Tendon Transfer: Is Arthrodesis Necessary?

Recurrence of deformity following soft tissue reconstruction, including tendon transfer, in the context of treatment for stage II flatfoot deformity represents a challenging problem for even the most experienced foot and ankle surgeon. The goals of treatment in flexible flatfoot should include restoration of alignment and alleviation of pain while minimizing stiffness, maintaining motion, and avoiding overcorrection. Arthrodesis should be reserved for fixed deformities or flexible deformities failing soft tissue reconstruction, including tendon transfer, where revision with extra-articular procedures is unlikely to facilitate adequate realignment and resolution of symptoms.
