Key Points 1. When looking at outcomes after adult living donor (LD) liver transplantation, both donor and recipient outcomes must be examined. 2. The estimated risk for mortality after right-lobe donation is 0.5%. The risk for complications is approximately 10% to 20%. 3. Current national data show slightly inferior patient and graft survival rates with LD versus deceased donor transplantation. The difference may be caused by a greater incidence of surgical complications after LD liver transplantation. 4. The main surgical complication in the donor and recipient remains biliary leak.
