Role of liver transplantation in Wilson's disease

Liver transplantation (LT) is the sole etiological therapy for Wilson's disease (WD). LT is the treatment of choice for WD presenting as fulminant hepatic failure and for WD patients with end-stage cirrhosis. Its effect on the neurological manifestations of the disease has been assessed in a small number of patients. WE summarized the data available from the literature on WD patients with neurological symptoms, who underwent LT; adding an unpublished personal observation, obtaining a sample of 40 patients. The transplant cured or improved the neurological symptoms in 32 of them (=80%). This leads to the question of whether liver transplantation could also be offered to patients with neurological Wilson's disease brit stable liver function. Given the still high mortality after liver transplantation, the neurological status in WD patients with stable liver functions cannot be in general an indication for LI: which in these cases has to be still considered experimental. Notwithstanding this, LT could be entertained in patients with severely disabling medically intractable neurological deterioration, informing them exhaustively on the surgical risk.
