Lateral ventricular localization of hydatid cyst is rarely seen. The preoperative diagnosis and planning of the surgery is very important in this disease. Surgical removal of the cerebral hydatid cyst is most effective treatment. The aim of surgery for cerebral hydatid cyst is total excision without rupture. Dowling's technique is reliable treatment of hydatid cyst into the lateral ventricle. If the cyst ruptures into the lateral ventricle during surgery, aspirate should be done to prevent spillage of cyst fluid and irrigation with hypertonic saline should not be employed. In this report, we present an adult patient with a hydatid cyst into the lateral ventricle and we suggest a few surgical points for successful birth of hydatid cyst into the lateral ventricle with Dowling's technique.
