Cryptococcal Meningitis in a Fingolimod-Treated Patient Positive Antigen Test a Year Before Onset

Fingolimod, a sphingosine 1-phosphate receptor modulator, has proven high efficacy for relapsing-remitting multiple sclerosis (MS)1 and is currently widely used for MS treatment. Fingolimod is classified as a second-line treatment2 because of adverse side effects, including bradyarrhythmia, macular edema, and liver dysfunction, as well as fatal infections due to herpes virus, John Cunningham virus (progressive multifocal leukoencephalopathy), and cryptococcus, but it is available as a first-line treatment in Japan. Herein, we report a patient with MS who received fingolimod treatment for 6 years and subsequently developed cryptococcal meningitis (CM). Retrospective testing of stored sera revealed positive conversion of cryptococcal antigen 1 year before the onset of clinical meningitis.
