The anatomy of the ulnar nerve is described from its origin at the brachial plexus to its termination in the hand and digits. The critical anatomy surrounding the cubital tunnel and Guyon canal is emphasized, and clinically relevant anatomic variations, muscle anomalies, and peripheral nerve anastomoses are described. The ulnar nerve originates from the ventral rami of the C8 and T I nerve roots, which unite to form the lower trunk of the brachial plexus. The lower trunk then divides into an anterior and posterior division with the fibers of the ulnar nerve in the anterior division, which becomes the medial cord. The cords are named according to their position relative to the axillary artery. The ulnar nerve exits the brachial plexus as a terminal branch of the medial cord and travels from the axilla into the medial aspect of the anterior compartment of the upper arm.
