The restless legs syndrome (RLS) is a common cause of disturbed sleep, and may have a marked effect on the patient's quality of life and mental health. Although the aetiology of the condition is still not fully understood, the dopaminergic system, iron metabolism and the opioidergic system appear to have a major role to play. While levodopa and dopamine agonists represent the treatment of choice, anticonvulsants and opioidergic drugs are also employed. Before, during and after anaesthesia a number of particulars need to be considered, and these are discussed in this article.
