The effect of nutritional supplementation on quality of life in people living with HIV: a randomised controlled trial

摘要
ObjectiveTo determine the effects of lipid-based nutrient supplements (LNS) on the quality of life of people living with HIV (PLHIV) during the first 3 months of antiretroviral treatment (ART) and to investigate the effects of timing of supplementation by comparing with supplementation during the subsequent 3 months. MethodsA randomised controlled trial was conducted in three ART clinics within public health facilities in Jimma, Ethiopia. Participants were PLHIV eligible to start ART with body mass index >17 kg/m(2) and given daily supplements of 200 g of LNS containing whey or soya either during the first 3 months or the subsequent months of ART. The outcome was measured in terms of total quality-of-life scores on the adapted version of the WHOQOL-HIV-BREF assessed at baseline, three and six months. ResultsOf the 282 participants, 186 (66.0%) were women. The mean age (SD) was 32.8 (9.0) years, and the mean (SD) total quality-of-life score was 82.0 (14.8) at baseline assessment. At 3 months, participants who received LNS showed better quality of life than those who only received ART without LNS ( = 6.2, 95% CI: 2.9: 9.6). At 6 months, there was no difference in total quality-of-life score between the early and delayed supplementation groups ( = 3.0, 95% CI: -0.4: 6.4). However, the early supplementation group showed higher scores on the social and spirituality domains than the delayed group. ConclusionsLNS given during the first three months of ART improves the quality of life of PLHIV. ObjectifDeterminer les effets de supplements nutritionnels a base de lipides (SNL) sur la qualite de vie des personnes vivant avec le VIH (PVVIH) au cours des 3 premiers mois de traitement antiretroviral (ART), et etudier les effets de cette periode de la supplementation par rapport a la supplementation au cours des 3 mois suivants. MethodesEssai controle randomise dans trois cliniques ART dans les etablissements de sante publique a Jimma, en Ethiopie. Les participants etaient des PVVIH eligibles pour commencer l'ART avec un indice de masse corporelle > 17 kg/m(2) et recevant des supplements quotidiens de 200 grammes de SNL contenant du lactoserum ou de soja, soit au cours des 3 premiers mois ou les mois suivants l'ART. Le resultat a ete mesure en termes de qualite totale des scores de la vie sur la version adaptee du WHOQOL-VIH- BREF evalue au depart, a 3 et a 6 mois. ResultatsSur les 282 participants, 186 (66,0%) etaient des femmes. L'age moyen (deviation standard SD) etait de 32,8 ( 9,0) ans, et la moyenne (SD) de la qualite totale du score de vie etait de 82,0 ( 14,8) lors de l'evaluation initiale. A 3 mois, les participants qui ont recu le SNL ont montre une meilleure qualite de vie que ceux qui ont recu uniquement l'ART sans SNL [ = 6,2, intervalle de confiance a 95%: 2,9- 9,6]. A 6 mois, il n'y avait pas de difference dans la qualite totale du score de la vie entre les groupes de supplementation precoce et tardive [ = 3,0; IC95%: -0,4-6,4]. Cependant, le groupe de supplementation precoce a montre des scores plus eleves sur les domaines sociaux et de spiritualite que le groupe tardif. ConclusionsLes SNL donnes au cours des 4 premiers mois de l'ART ameliorent la qualite de vie des PVVIH. ObjetivoDeterminar los efectos de los suplementos nutricionales de base lipidica (LNS) sobre la calidad de vida de personas viviendo con VIH (PVVIH) tomados durante los tres primeros meses de tratamiento antirretroviral (TAR), e investigar los efectos del momento de la suplementacion, comparando con la suplementacion durante los 3 meses siguientes. MetodosEnsayo aleatorizado y controlado en tres centros de TAR dentro del sistema sanitario publico en Jimma, Etiopia. Los participantes eran PVVIH que cumplian criterios para iniciar TAR con un indice de masa corporal >17kg/m(2) a los que se les dieron 200 gramos diarios de suplementos LNS que contenian proteina de suero de leche o soja, bien durante los 3 primeros meses o los meses subsecuentes de TAR. Los resultados se midieron en terminos de las puntuaciones totales de calidad de vida utilizando la version adaptada del instrumento de la OMS para evaluar la calidad de vida de personas viviendo con VIH, el WHOQOL-HIV-BREF, en evaluaciones realizadas al comienzo del estudio y a los 3 y 6 meses. ResultadosDe los 282 participantes, 186 (66.0%) eran mujeres. La edad media era de 32.8 (+/- 9.0) anos y la media de la desviacion estandar del puntaje total de calidad de vida era de 82.0 (+/- 14.8) al comienzo del estudio. A los 3 meses, los participantes que recibieron LNS mostraron una mejor calidad de vida que aquellos que solo recibieron TAR sin LNS [=6.2, 95% intervalo de confianza (IC): 2.9: 9.6]. A los 6 meses, no habia diferencia en el puntaje total de calidad de vida entre los grupos con suplementacion temprana y tardia [=3.0, IC 95%: -0.4: 6.4]. Sin embargo, el grupo con suplementacion temprana mostraba puntajes superiores en los dominios social y espiritual que el grupo tardio. ConclusionesLNS durante los primeros 4 meses de TAR mejora la calidad de vida de las PVVIH.
