THREE-LEVEL PRECISION CONGNITIVE INTERVENTION ON SELF-MANAGEMENT LEVEL OF PATIENTS WITH CHRONIC DISEASES

Objective: To explore the effect of three-level precise cognitive intervention On self-management level of patients with chronic diseases. Methods: According to the principle of voluntary selection, The 120 patients in chronic diseasesmainly took care of giving three-level precise cognitive intervention on the basis of routine health education. doctors from different departments of chronic diseases were selected to form a follow-up group. Before and after the follow-up, the self-management ability of chronic disease patients was evaluated and the career status of the primary doctors was studied. Results: The self-management ability of chronic disease patients, the scores of career evaluation of primary doctors were significantly improved, and the differences were significant in before and after comparison. Conclusion: The design and application of three-level follow-up system play a positive role in promoting the improvement of life and quality of life of patients with chronic diseases it can improve self-management ability of chronic disease patients and increase career satisfaction of primary doctors.
