Heart dysrhythmies during epileptic seizures Epileptic seizures can propagate to the cortical regions controlling the autonomic nervous system. The clinical consequences vary according to a preferential activation of sympathetic or para-sympathetic system. Tachycardia is usually observed during a partial seizure, whereas ictal bradycardia or asystole are rare events. Ictal asytole can provoke a syncope and head trauma. In addition, it was proposed that ictal asystole may also be at the origin of sudden unexplained death of epileptic patients (SUDEP). The weak localizing and lateralizing value of ictal heart dysrhythmy can be explained by the widespread distribution of cortical areas controlling the vegetative system. Both neurologists and cardiologists must take care of these patients. Treatment must be tailored for each patient, including systematically, as a first line, antiepileptic drugs. A pacemaker can be proposed to the patients with pharmacoresistant epilepsy and remaining ictal syncopes.
