In a randomized, prospective study we compared the results of open and laparoscopic tension-free mesh repair for elective hemiorrhaphy. Laparoscopic hernia repair produced less patient discomfort, but this did not translate into an earlier return to unrestricted full activities. On follow-up there were significantly more hernia recurrences in the laparoscopic mesh repair. Open mesh inguinal herniorrhaphy is the more effective hernia repair and allows equally early return to full activities.
