We are reporting a clinical case of paraparesis in a 75 years old woman, Paresis was initially attributed to diabetic polineuropathy. Neurophysiological studies suggested a lesion of the medullary cone or the filum terminale which was confirmed by a MR study showing a cystic dilatation of the medullary cone, The patient was operated and the histological study showed a neuroepitelial cyst, The clinical presentation in our patient with terminal siringohydromielia is rather inusual.
