Preeclampsia: a risk-factor analysis

Introduction: Thrombophilia and preeclampsia are related to preterm deliveries and fetal death due to placental thrombosis (PT). Objective: To evaluate clinical and laboratory data as predictors of preeclampsia. Methods: Medical records of 117 women with a preterm delivery or a 3(rd) trimester fetal loss due to PT were reviewed, comparing thrombophilic women with normotensive pregnancies to patients with severe preeclampsia. Analyzed data included weight accrual rate (WAR), defined as weight gain from 18 years old to the beginning of pregnancy divided by elapsed years. Results: Independent predictors were WAR>0.8 kg/years (>1.8 lb/years), oligoamenorrhea, acanthosis and first trimester or pre-pregnancy plasmatic triglycerides levels >150 mg/dL. Conclusion: The presence of 2 out 4 predictors showed a hazard ratio of 48 (95% CI [15-150]; p<0.001) to develop preeclampsia (88% specificity, 82% sensitivity, c-statistic of 87 +/- 4%; p = 0.0001).
