Intraocular lenses should block UV radiation and violet but not blue light

Intraocular lens chromophore design balances visual performance with macular protection. Acute UV-blue phototoxicity has an actionspectrum that increases with decreasing wavelength in an aphakic eye. Acute blue-green phototoxicity has an action spectrum that peaks at 507 nm, similar to scotopic luminous sensitivity. UV radiation and violet light (400-440 nm) have substantial potential acute UV-blue phototoxicity and provide negligible visual information, so it is prudent to block them with IOL chromophores. Conversely, blue light (440-500 nm) has significant potential acute UV-blue phototoxicity, but it is vital for scotopic vision. The AcrySof Natural IOL absorbs UVR as well as violet, blue, and even some green light. Restricting blue light limits rod-mediated visual function, which declines with aging and is even worse in people with AMD and diabetic retinopathy. No IOL provides significant protection against blue-green phototoxicity so protective sunglasses should be worn in bright environments regardless of IOL choice.
