Experiences and acceptance of intimate partner violence: associations with sexually transmitted infection symptoms and ability to negotiate sexual safety among young Liberian women

Women who experience intimate partner violence (IPV) may be at elevated risk for poor sexual health outcomes, including sexually transmitted infections (STIs). This association, however, has not been consistently demonstrated in low-income or post-conflict countries. Furthermore, the role that attitudes towards IPV play in sexual-health outcomes and behaviour has rarely been examined. We examined associations between IPV experiences, accepting attitudes towards physical IPV, and sexual-health and behavioural outcomes among 592 young women in post-conflict Liberia. Participants' experiences with either moderate or severe physical violence or sexual violence were common. Additionally, accepting attitudes towards physical IPV were positively associated with reporting STI symptoms, IPV experiences and the ability to negotiate safe sex. Findings suggest that for sexual-health-promotion and risk-reduction-intervention efforts to achieve full impact, interventions must address the contextual influence of violence, including individual attitudes toward IPV. Las mujeres que sufren violencia de pareja pueden estar en grave peligro de padecer una salud sexual deficiente, incluyendo infecciones de transmision sexual (ITS). Sin embargo, esta asociacion no se ha demostrado sistematicamente en paises de bajos ingresos o en situacion de posconflicto. Ademas, pocas veces se ha analizado como afectan en el comportamiento y la salud sexuales las actitudes hacia la violencia de pareja. Aqui examinamos los vinculos entre las experiencias de violencia de pareja, las actitudes que aceptan la violencia fisica de pareja, y las consecuencias en la salud y el comportamiento en 592 mujeres jovenes en la Liberia posguerra. Entre las participantes era habitual haber sufrido violencia fisica moderada o grave o violencia sexual. Ademas, las actitudes de aceptacion hacia la violencia fisica de pareja estaban directamente relacionadas con los sintomas de ITS, las experiencias de violencia de pareja y la capacidad para negociar relaciones sexuales seguras. Los resultados indican que para que tengan exito los programas de fomento de la salud sexual y reduccion de riesgos, se debe abordar la influencia contextual de la violencia, incluyendo actitudes individuales hacia la violencia de pareja. Les femmes qui subissent des violences exercees par leurs partenaires intimes peuvent etre fortement exposees a des problemes de sante sexuelle, parmi lesquels les infections sexuellement transmises (IST). Cependant cette association n'est pas demontree de maniere constante dans les pays a ressources limitees ou sortant d'un conflit; de plus, l'impact des attitudes vis-a-vis de ce type de violences sur la sante sexuelle et les comportements sexuels a ete rarement examine. Nous avons examine les associations entre les experiences de violences exercees par les partenaires intimes, les attitudes de comprehension vis-a-vis de la violence physique exercee dans ce contexte, et l'etat de sante sexuelle et les comportements sexuels parmi 592 jeunes femmes vivant dans le Liberia d'apres guerre. Les experiences de violences physiques ou sexuelles moderees ou graves etaient frequentes parmi ces femmes. De plus, les attitudes de comprehension vis-a-vis de ces violences etaient positivement associees aux declarations de symptomes d'IST, aux experiences de violences et a la capacite de negocier le sexe a moindre risque. Les resultats suggerent que les programmes de promotion de la sante sexuelle et de reduction des risques ne peuvent avoir un impact majeur que si les interventions tiennent compte de l'influence contextuelle de la violence, y compris des attitudes individuelles vis-a-vis de la violence exercee par les partenaires intimes.
