An Unusual Optic Neuropathy Case Caused by Sinonasal Undifferentiated Carcinoma

A 27-year-old male presented with progressively blurred vision in his right eye for 2 weeks without epistaxis, nasal obstruction, facial pain, rhinorrhea, or headache. Magnetic resonance imaging revealed a large enhancing mass lesion (47 55 62 mm) involving the cribriform plate, the ethmoid sinuses bilaterally, and the left upper nasal cavity with intracranial extension to the frontal lobe and compression of the right proximal optic nerve at the orbital apex. The pathologic findings obtained by incision biopsy were consistent with sinonasal undifferentiated carcinoma. Therefore, a diagnosis of compressive optic neuropathy caused by sinonasal undifferentiated carcinoma was considered and a treatment of concurrent chemotherapy and radiotherapy was initiated. The visual acuity of the subject improved from light perception to 6/20 after treatment.
