Hapten-/Prohapten- and p-i-mechanisms are described as pathomechanisms to explain sensitization to small molecular weight compounds leading to the precipitation of drug allergic reactions. The influence of xenobiotica metabolism - in particular by cytochrome P450 and transferases - on these reactions is presented taking nevirapine, sulfamethoxazole, allopurinol, and anticonvulsiva as examples. Furthermore, the possible impact on the predictivity of drug allergy as well on diagnostic procedures is discussed. © 2017 Dustri-Verlag Dr. Karl Feistle.
