Comminuted proximal and distal femur fractures are amenable to plate fixation. However, complication rates are high when anatomic reconstruction has been achieved. Bone grafting can further devascularize the bone. A solution to the problem is ''biological'' fixation which incorporates indirect reduction and bridging osteosynthesis. By using radiographs, animation and explanatory film sequences, the video provides a precise description of the technique of ''biological'' fixation and a step-by-step review of this atraumatic procedure.
