Many surgical techniques aimed to obtain an anatomical anterior cruciate ligament reconstruction using the hamstrings tendons are described in the literature. All of these techniques encompass the detachment of the tibial insertion of the hamstrings tendons, thus damaging the neurovascular supply of the tendons and resulting in lower residual proprio-ceptivity and worse ligamentization of the graft. This article describes a new technique that permits an anatomic single-bundle ACL reconstruction using distally inserted doubled hamstrings tendons fixed at the femoral level with a second-generation cortical suspensory device.
