Problematic clinical situations are discussed in the 'Clinical Dilemma' section. The Editors wish to encourage the BJS readership to play a part here. If you have a 'dilemma' which can be expressed as a question (not more than 40 words), please let us know by contacting the BJS office. You may even have thoughts on a suitable expert to voice an opinion. The most interesting problems will be followed up in this section of the Journal in the coming months. In this issue of BJS a dilemma related to thyroid surgery is highlighted what to do if the recurrent laryngeal nerve is difficult (or impossible) to find. How should this situation be handled? Professor Malcolm Wheeler of Cardiff replies.
