摘要：

The diverse morphology of the root canal system of the mandibular premolar is a challenge for the clinician to clean, shape and obturate. A case report of a mandibular second premolar with four fused roots which separated at different levels along the apical third of its root and underwent endodontic treatment is presented, along with a review of the literature.
