A case of a young man with myocarditis simulating acute coronary syndrome is reported. The possibility of vasospasm is discussed. The use of cardiac magnetic resonance imaging (MRI) is highlighted. J Cardiovasc Med 10:264-266 (C) 2009 Italian Federation of Cardiology.
