摘要
One of the greatest challenges in geriatric medicine is to provide optimal care for older adults with multiple chronic conditions (comorbidity). The risk of developing concomitant chronic illnesses and functional limitations escalates with aging. Older patients with comorbidity have an increased risk of adverse outcomes when admitted to the hospital. Comprehensive geriatric assessment is at present the best model for identification and management of the complexity that characterizes these patients.
