A 42-year-old man had intermittent right upper quadrant pain. Initial ultrasound examination showed a mass in the posterior segment of the right lobe of the liver. A tagged red blood cell scan was obtained to evaluate this lesion. No abnormal uptake was present on either planar or SPECT images. MR imaging showed that the lesion was hyperintense on heavily T2-weighted images and had a classic enhancement pattern (peripheral nodular pattern). Large nonthrombosed hepatic hemangiomas may be present despite the lack of uptake on a tagged red blood cell scan.
