In recent years, following worldwide changes in the prevalence, distribution and activity of dental caries in children and changes in the delivery of dental services, there has been much debate about related changes in the dental health status of children in the UK. This has been stimulated by observations in practice, community clinic or hospital environments and from the results of national epidemiological surveys. Sometimes it has not been at all clear if we are receiving good or bad news.(1) The aim of this paper (which is entirely a personal view) is to highlight some pertinent aspects of the overlapping issues specifically surrounding caries status and caries management in UK children in order to illuminate the question: do we understand which children need and get appropriate dental care? I will consider these issues under four separate headings: 1 Do we understand dental care provision for children? 2 Which children need dental care? 3 Which children get dental care? 4 What is appropriate for the future?
