Mitral annular calcification (MAC) is the result of an underlying atherosclerotic disease and is often associated with an increased cardiovascular morbidity and mortality. Surgery is necessary when patients develop symptoms due to valvular heart disease, and the mechanism of disease is usually mitral valve insufficiency. This manuscript discusses the histopathological aspects related to MAC and describes surgical strategies used in debridement and reconstruction of the annulus followed by repair or replacement of the mitral valve. In addition to the typical strategies used, rare and rather experimental surgical techniques are also discussed.
