For the last 10 years, end-of induction treatment MRD negative rate has been remarkably increased, Rituximab maintenance treatment interest has been confirmed, and breakthrough has been made in relapsed patients management with BTK inhibitors (BTKi) advent. While new generations of BTKi molecules are coming soon, new targets (such as BCL2 pathway) and development of CAR T-cells treatment increase the therapeutic landscape. Despite these new options persist high risk patients with rapid progression course, difficult to identify while untreated as tumoral heterogeneity of this lymphoma appears wider and wider as understanding in this field is processing. With all these new options, a paradigm shift might be seen in a few years for untreated mantle cell lymphoma patients. Will new combinations including targeted therapies question autologous stem cell transplantation indication (especially for MRD negative patients)? Is chemo-free treatment the future of untreated elderly patients (and young)?
