Evidence-based perioperative management of colorectal carcinoma

This article summarizes the evidence on perioperative management for oncological colorectal resections as a narrative review. The main sources were the colorectal enhanced recovery after surgery (ERAS (R)) guidelines and the perioperative management of gastrointestinal tumors (POMGAT) guidelines. In addition, a literature search was performed in MEDLINE via PubMed (R), in which meta-analyses and randomized controlled trials (RCT) were preferentially sought. According to the current state of the science, the perioperative management of oncological colorectal surgery represents a complex multimodal multidisciplinary construct that takes the various perioperative stages and disciplines into account. As now confirmed by high-quality studies, multimodal perioperative management can significantly improve postoperative outcomes after oncological colorectal surgery. The evidence on the effect of ERAS (R) on oncological outcomes is still conflicting; however, some studies suggest a survival benefit, among others.
