Some patients with liver cirrhosis develop (multi-)organ failure in the setting of acute decompensation. The presenting syndrome is characterized by a state of severe systemic inflammation and is associated with high mortality. This syndrome is referred to as acute-on-chronic liver failure (ACLF) and is thought to be a response to certain triggers like alcohol binge drinking or infections. Although there are differences regarding diagnostic criteria proposed by international consortia, it is universally acknowledged as a distinct clinical entity. To diagnose ACLF, scores have been developed with the aim to assess dysfunction across organ systems. There is currently no specific treatment for ACLF. Hence, identifying and addressing precipitating events is crucial. Since liver transplantation is also an option in ACLF, early consultation with a transplant center is advisable.
