Since 1991 to 2010, data on total ankle replacement (TAR) reported a relative increase of 6.71-fold, along with a simultaneous 15.6% reduction of ankle arthrodesis (US data).1 TAR used to be a challenging treatment only for selected patients. Nowadays, TAR is gold standard treatment of ankle arthritis in high-volume centers with surgeons at the end of their learning curve. TAR has been originally developed through an anterior approach. It allows direct visualization of the coronal alignment and an easy access to the medial-lateral edges Although it is widely considered as the easiest and the most popular approach, anterior approach presents some limitations, especially when addressing sagittal malalignment deformities.
