In January 2023, the American Heart Association and American Stroke Association introduced the idea of implementing a brain date during the International Stroke Conference held in February 2023. The idea was to foster discussion about top-ics that could advance stroke care while fomenting networking and collaborations. Champions were in-vited to lead such groups with different formats. A group was formed for an open discussion about the current state of clinical care for patients with covert (subclinical) cerebral small vessel disease (cSVD) including screening, prevention, and management of these patients while paying attention to countries with limited resources. The group included 5 attend-ees approaching this topic from different perspec-tives: a neurology physician–resident in training, an early career vascular neurologist, a seasoned clinician practicing at a country in a context of limited resources, a clinician–scientist and division director of stroke and neurocritical care at an academic in-stitution with expertise in cSVD, and an established clinician and a director of a stroke center. Because high-quality evidence is currently lacking to support specific recommendations on the questions posed in our discussion, the conclusions presented in this article primarily aim to increase awareness, promote recognition, and stimulate future research to address the current knowledge gaps in screening, prevention, and management of covert cSVD. © 2023 The Authors.
