We report on a 79-year-old female patient with focal epilepsy and cognitive status epilepticus suffering from progressive cognitive slowing and extrapyramidal symptoms during treatment with valproic acid (VPA). A switch from VPA to lacosamide led to impressive but gradual clinical improvement over months. We show that a complete remission of symptoms following discontinuation of VPA can be expected and discuss pitfalls of an early diagnosis and treatment in cases with comorbidities.
