Heart heart failure with preserved ejection fraction - Update 2022.. Until now no treatment had convincingly shown a reduction in mortality and morbidity in heart failure with preserved ejection fraction (HFpEF). The EMPEROR -Preserved trial, empagliflozin in HFpEF, is the first positive outcome study that reduced the combined risk for cardiovascular death or hospitalization, whether suffering from type 2 diabetes or not [1]. The DELIVER study, dapagliflozin in HFpEF, significantly reduced the primary endpoint of cardiovascular death or worsening in heart failure with mildly reduced ejection fraction (HFmrEF) or HFpEF [2, 3]. Current ESC guidelines further recommend the use of sacubitril/valsar tan and spironolactone in HFmrEF patients. Supporting data were provided by a sub-group analysis of the PARAGON-HF study showing a reduction in HF-related hospitalizations and a meta-analysis of the PARADIGM-HF and PARAGON-HF studies showing a reduction in cardiovascular deaths and HF-related hospitalizations
