Gamma钉与InterTan髓内钉固定治疗股骨转子间骨折的生物力学性能比较

摘要
目的比较第3代Gamma钉与InterTan髓内钉固定治疗股骨转子间骨折的生物力学性能。方法选用12根第4代人工合成模型骨,随机分为A、B、C、D 4组(n=3)。A、B组制作稳定型股骨转子间骨折(AO分型为31-A1.1型)模型,C、D组制作不稳定型股骨转子间骨折(AO分型为31-A2.3型)模型,分别用Gamma钉(A、C组)和InterTan髓内钉(B、D组)固定并进行生物力学测试。结果对于稳定型股骨转子间骨折模型:随着轴向压力逐级增加,头下沉位移呈增大趋势;随着扭转角度的逐渐增大,扭矩也呈逐渐增大趋势,A组与B组的头下沉位移、扭矩比较差异均有统计学意义(P<0.05)。A组与B组的最大破坏载荷平均分别为(5 855.5±627.8)、(5 547.1±365.4)N,差异无统计学意义(P>0.05)。对于不稳定型股骨转子间骨折:当轴向载荷为500、600、700 N时,C组与D组的头下沉位移比较差异均无统计学意义(P>0.05);而当轴向载荷大于800 N时,C组与D组的头下沉位移比较差异有统计学意义(P<0.05)。随着扭转角度的逐渐增大,C组与D组的扭矩也呈增大趋势,同一扭转角度下两组之间扭矩比较差异均有统计学意义(P<0.05)。C组与D组的最大破坏载荷平均分别为(2 781.5±600.6)、(3150.5±633.8)N,差异有统计学意义(P<0.05)。结论对于稳定型股骨转子间骨折模型,InterTan髓内钉和Gamma钉的生物力学性能相差不大;在不稳定型股骨转子间骨折模型中,InterrTan髓内钉的生物力学稳定性明显优于Gamma钉。
