云南省首例人感染G4基因型欧亚类禽H1N1猪流感病毒病原学特征分析

摘要
2020年云南发现了首例人感染G4基因型欧亚类禽H1N1(EA H1N1)猪流感病毒,针对分离到的A/YunnanMengzi/1462/2020(H1N1v)病毒分析其血凝素(HA)和神经氨酸酶(NA)氨基酸变异位点、生物学耐药、受体亲和力及抗原性变异特征,并对2020-2021年度季节性流感疫苗对该病毒的交叉保护效果进行评价。结果显示,EA H1N1猪流感病毒A/Yunnan-Mengzi/1462/2020(H1N1v)与世界卫生组织推荐的疫苗株A/Hunan/42443/2015(H1N1v)HA和NA氨基酸位点同源性分别为97.9%和97.4%;A/Yunnan-Mengzi/1462/2020(H1N1v)病毒对神经氨酸酶抑制剂敏感,以结合人流感病毒唾液酸受体α2,6为主,与疫苗株抗原性存在8倍以上差异。接种季节性流感疫苗的儿童、成人和老年组人群针对季节性流感疫苗株A/Guangdong-Maonan/SWL1536/2019(H1N1pdm)抗体滴度≥40者占比分别为80.0%、76.7%和63.3%;而对A/Yunnan-Mengzi/1462/2020(H1N1v)抗体滴度仅为13.3%、26.7%和6.7%。本研究提示接种季节性流感疫苗后产生的抗体对G4基因型EA H1N1病毒不能提供有效保护,该病毒与目前EA H1N1病毒疫苗株在氨基酸位点和抗原性上存在显著差异,并且保持了结合人流感病毒唾液酸受体的特性,研究结果提示需要构建新的疫苗株。
