股骨颈骨折分型与治疗

摘要
目前最常用的股骨颈骨折分型为Pauwels分型、Garden分型和AO分型,但均不适用于临床指导手术。股骨颈骨折治疗分为保守治疗和手术治疗。在需手术治疗的股骨颈骨折中,根据骨折端是否移位和骨折线偏水平或垂直,选择闭合复位内固定(CRIF)、切开复位内固定(ORIF)、半髋关节置换术(HA)和全髋关节置换术(THA)。骨折线偏垂直型股骨颈骨折治疗除常规使用的空心加压螺钉(CCS)内固定、HA、THA外,近年来采用CCS联合锁定钢板(LP)、滑动髋螺钉(DHS)联合防旋钉(DS)、新型股骨颈锁定钢板(FNLP)、髓内钉系统、经皮加压钢板(PCCP)等进行内固定也得到了较为普遍的认可与应用。对于>80岁的股骨颈骨折患者,一般建议髋关节置换;对于年龄<65岁的无移位股骨颈骨折患者,可行保守治疗或CRIF、ORIF;对于年龄65~80岁的股骨颈骨折患者,应遵循个体化治疗原则。
