哮喘患者外周血CD8 CD25 Foxp3 Treg细胞检测及其临床意义

目的:通过检测哮喘患者外周血CD8+ CD25+ Foxp3+ Treg(CD8+ Treg)细胞比例,探讨其在哮喘发生发展过程中的作用。方法:选择哮喘患者50例,其中轻中度哮喘25例,重度哮喘25例,并选择与患者年龄、性别匹配的25例健康者作为对照组。流式细胞仪分析CD8+ Treg细胞比例;RT-PCR检测外周血单个核细胞Foxp3 mRNA的表达;Luminex200检测血清IL-33、IL-1β和转化生长因子β1(TGF-β1)浓度。肺功能测试仪测定第1秒用力呼气容积(FEV1)、呼气峰流速(PEF)。结果:重度哮喘组和轻中度哮喘组CD8+ Treg比例分别为[2.32(1.243.63)%]和[4.13(2.055.41)%],两组均低于对照组[6.48(5.216.92)%],且重度哮喘组降低更显著(P<0.05)。与对照组相比,重度哮喘组及轻中度哮喘组血清IL-33浓度均明显升高,且重度哮喘组升高更明显(P<0.05);与对照组及轻中度哮喘组相比,重度哮喘组TGF-β1和IL-1β明显升高(P<0.05);哮喘患者CD8+ Treg细胞比例与FEV1(r=0.411,P=0.003)、PEF(r=0.441,P=0.001)呈正相关。ROC曲线分析显示,CD8+ Treg诊断哮喘具有较高的特异性和敏感性。结论:哮喘患者外周血CD8+ Treg细胞比例降低,在疾病发生发展过程中发挥重要作用。
