血清miRNA-10a与IL-35水平对脓毒症诱导急性肾损伤患者预后评估价值

摘要
目的评估血清微小RNA-10a(miRNA-10a)与白细胞介素-35(IL-35)水平对脓毒症诱导急性肾损伤(AKI)患者病情程度及短期预后结局的预测价值。方法 90例脓毒症患者,按有无伴发AKI分为观察组42例和对照组48例。在入院即刻、治疗第3、7、14、28天分别检测并比较两组患者外周血清miRNA-10a、IL-35、降钙素原(PCT)、肌酐(Scr)、肾损伤分子1(KIM-1)、胱抑素C(Cys-C)、序贯器官衰竭评分(SOFA)指标;采用Pearson相关分析分别评估血清miRNA-10a、IL-35与PCT、Scr、KIM-1、Cys-C及SOFA评分的相关性;随访3个月,根据预后结局分为生存组56例和死亡组34例,比较两组血清miRNA-10a及IL-35水平;应用多元Logistic回归分析上述各临床指标与生存率的相关性;绘制受试者工作特征曲线(ROC)评估血清miRNA-10a联合IL-35水平对脓毒症诱导AKI患者短期预后的判断价值。结果对照组不同时间点血清PCT水平、SOFA评分差异有统计学意义(P均<0.05),不同时间点血清miRNA-10a、IL-35、Scr、KIM-1、Cys-C水平差异无统计学意义(P均>0.05);观察组不同时间点血清miRNA-10a、IL-35、PCT、Scr、KIM-1、Cys-C水平及SOFA评分自治疗第三天开始随治疗时间延长而逐渐降低(P均<0.05),同一时间点观察组血清miRNA-10a、IL-35、Scr、KIM-1、Cys-C水平与对照组比较,差异有统计学意义[miRNA-10a:(3.34±0.53)比(1.09±0.32)、(3.89±0.73)比(1.15±0.37)、(3.68±0.59)比(1.10±0.40)、(3.25±0.47)比(1.12±0.42)、(2.85±0.51)比(1.07±0.39),P均<0.05;IL-35:(502.63±37.91)比(271.41±25.61)、(584.74±59.83)比(281.47±33.80)、(511.55±63.61)比(270.62±27.53)、(435.43±43.46)比(275.36±30.17)、(352.24±40.62)比(262.12±28.24),P均<0.05;Scr:(416.30±71.32)比(81.93±14.33)、(535.78±71.32)比(87.90±14.33)、(481.41±64.23)比(82.47±12.56)、(425.38±53.91)比(78.26±11.28)、(352.23±46.67)比(81.74±10.86),P均<0.05;KIM-1:(17.82±3.75)比(4.84±0.34)、(26.27±4.41)比(4.86±0.64)、(21.37±3.58)比(5.09±0.58)、(18.65±3.05)比(4.92±0.67)、(15.01±2.46)比(4.79±0.52),P均<0.05;Cys-C:(1.86±0.21)比(0.71±0.09)、(2.13±0.26)比(0.74±0.10)、(1.74±0.22)比(0.69±0.12)、(1.46±0.25)比(0.72±0.13)、(1.16±0.19)比(0.70±0.14),P均<0.05]。Pearson相关分析显示,脓毒症诱导AKI患者血清miRNA-10a、IL-35分别与血清KIM-1、Cys-C水平正相关(r=0.781、0.842,r=0.835、0.864,P均<0.05);随访3个月,生存组血清miRNA-10a、IL-35水平显著低于死亡组[miRNA-10a:(4.58±0.71)比(5.01±0.68),P<0.05;IL-35:(379.73±69.46)比(428.14±73.18),P<0.05];多元Logistic回归分析显示,血清miRNA-10a、IL-35为脓毒症诱导AKI患者预后的危险性因素(OR=3.157、2.426,P均<0.05);ROC曲线显示,血清miRNA-10a截断值为3.15,IL-35截断值为355pg/mL,联合检测血清miRNA-10a与IL-35水平预测脓毒症诱导AKI患者预后不良发生的AUC为0.852(P=0.032),灵敏度85.6%(95%CI:0.810~0.924),特异度87.9%(95%CI:0.826~0.941)。结论检测血清miRNA-10a及IL-35水平能较好地预测脓毒症诱导AKI患者的病情及短期预后。
