重复经颅磁刺激对脑卒中患者下肢痉挛和运动功能的影响研究

摘要
目的探讨低频重复经颅磁刺激(rTMS)对慢性脑卒中患者下肢痉挛状态、运动功能及运动神经元兴奋性的影响。方法选取2016年1月至2017年8月在该院住院的脑卒中患者140例为研究对象,分为组1和组2,每组70例。分别给予连续5drTMS真刺激和假刺激治疗,其中134例患者(每组67例)在经过1个月的平衡期后,交叉进行真、假刺激。分别在治疗前、治疗后和随访1周时,对患者进行改良Ashworth痉挛量表(MAS)、H反射、Fugl-meyer平衡评估量表(FMA)的下肢部分及步态评估(TUG)测试。结果只有在接受rTMS真刺激后,MAS评分才有明显改善(P<0.05),这种改善在接受rTMS真刺激后持续1周。时间与组别在FMA评分方差分析模型中存在明显交互作用(P<0.05)。两组在MAS评分和FMA评分比较差异有统计学意义(P<0.05)。两组的Hmax/Mmax和TUG测试无明显变化(P>0.05)。结论低频rTMS治疗有助于减少脑卒中患者的下肢痉挛状态,并改善运动功能。
