Impact of Family Pharmacist Intervention on Medication Adherence in Chronic Disease Patients: Mediated Effects Analysis Based on Community Pharmacy Data

This study evaluated the effectiveness of a family pharmacist team intervention on improving medication adherence among patients with chronic diseases. A total of 424 participants were randomly assigned to either an intervention group or a control group. The intervention group received structured support including medication guidance, health education, and self-management training for six months, while the control group received standard pharmacy services. Results showed that the intervention significantly improved medication adherence, with the average score rising from baseline after the intervention (6.89 versus 5.56), showing a large effect size (Cohen’s d = 0.98). Further analysis revealed that the improvement was partly mediated by enhanced health beliefs, increased medication knowledge, and better self-management ability, which together accounted for 68.8% of the total effect. These findings suggest that family pharmacist interventions can be an effective strategy for managing chronic diseases by addressing key psychological and behavioural factors. © 2025 IGI Global.
