Undiagnosed recurrent pleural effusion is a challenging condition to investigate. It can be due to various factors, such as infection, malignancy, and inflammatory processes. Athorough investigation is needed to rule out different causes, including malignant mesothelioma. Here, we report a 68-year-old female who presented with a history of recurrent pleural effusion, positive rheumatoid (RA) factor, and antinuclear antibody (ANA), along with no asbestos exposure history. It is an interesting case presentation of a mimicking dual etiology-inflammatory and malignant.
