Association between Serum Uric Acid Levels and Islet Beta-cell Function in Type 2 Diabetes Mellitus

Background:Type 2 diabetes mellitus (T2DM) is a disease characterized by insulin resistance (IR) and insufficiency. Serum uric acid (SUA) is a biomarker of diabetes risk, with Indian populations having varying levels due to diet, lifestyle, and genetic susceptibility. However, the relationship between UA and T2DM is contentious due to its association with obesity. The study explores the relationship between serum uric acid levels and islet beta-cell function in type 2 diabetes patients, estimating the correlation, examining factors such as age, gender, or body mass index (BMI), and estimating the correlation between uric acid levels and IR.Methods:A study at Santosh Medical College in Ghaziabad involved 167 patients over 18 years old with type 2 diabetes, diagnosed based on fasting plasma glucose levels, and glycated hemoglobin (HbA1c). The patients underwent detailed history and physical examination, including fasting blood glucose, lipid profile, serum urea, creatinine, uric acid, HbA1c, insulin sensitivity index, and islet beta-cell function indices.Results:The study involved 170 patients, with 84 (49.41%) being male and 86 (50.59%) being female. The mean lipid profile was similar across genders, with no significant differences in HbA1c, random blood sugar (RBS), total cholesterol (TC), triglyceride (TG), high-density lipoprotein-cholesterol (HDL-c), and homeostatic model assessment of IR (HOMA-IR). Smoking and alcohol consumption were significantly higher in males, while family history of T2DM was comparable between both genders. The relationship between SUA quartiles and other parameters and HOMA-IR in males was not significantly different, while in females, the mean age, BMI, waist circumference, waist-hip ratio, HbA1c, RBS, TC, TG, HDL-c, low-density lipoprotein-cholesterol, HOMA-IR, and creatinine were not significantly different. The correlation of HOMA-IR and uric acid was positive but not significantly associated.Conclusion:The study found a significant positive correlation between serum uric acid levels and IR in males and females and a positive but nonsignificant correlation with BMI, suggesting that SUA could predict islet beta-cell activity. Contexte:Le diabè te sucré de type 2 (DT2) est une maladie caracté risé e par une ré sistance à l'insuline (RI) et une insuffisance. L'acide urique sé rique (AUS) est un biomarqueur du risque de diabè te, les populations indiennes ayant des niveaux variables en raison du ré gime alimentaire, du mode de vie et de la susceptibilité gé né tique. Cependant, la relation entre l'UA et le DT2 est controversé e en raison de son association avec l'obé sité. L'é tude explore la relation entre les niveaux d'acide urique sé rique et la fonction des cellules bê ta des î lots chez les patients atteints de diabè te de type 2, en estimant la corré lation, en examinant des facteurs tels que l'â ge, le sexe ou l'indice de masse corporelle (IMC) et en estimant la corré lation entre les niveaux d'acide urique et l'IR.Mé thodes:Une é tude au Santosh Medical College de Ghaziabad a porté sur 167 patients de plus de 18 ans atteints de diabè te de type 2, diagnostiqué s sur la base des taux de glucose plasmatique à jeun et de l'hé moglobine glyqué e (HbA1c). Les patients ont subi une anamnè se dé taillé e et un examen physique, notamment la glycé mie à jeun, le profil lipidique, l'uré e sé rique, la cré atinine, l'acide urique, l'HbA1c, l'indice de sensibilité à l'insuline et les indices de fonction des cellules bê ta des î lots.Ré sultats:Au total, 170 patients, 84 (49,41 %) é taient des hommes et 86 (50,59 %) des femmes. Le profil lipidique moyen é tait similaire entre les sexes, sans diffé rences significatives en termes d'HbA1c, de glycé mie alé atoire (RBS), de cholesté rol total (TC), de triglycé rides (TG), de cholesté rol à lipoproté ines de haute densité (HDL-c) et d'é valuation du modè le homé ostatique de l'IR (HOMA-IR). La consommation de tabac et d'alcool é tait significativement plus é levé e chez les hommes, tandis que les anté cé dents familiaux de diabè te de type 2 é taient comparables entre les deux sexes. La relation entre les quartiles SUA et d'autres paramè tres et HOMA-IR chez les hommes n'é tait pas significativement diffé rente, tandis que chez les femmes, l'â ge moyen, l'IMC, le tour de taille, le rapport taille-hanches, l'HbA1c, le RBS, le TC, le TG, le HDL-c, le cholesté rol à lipoproté ines de basse densité, HOMA-IR et la cré atinine n'é taient pas significativement diffé rents. La corré lation entre HOMA-IR et l'acide urique é tait positive mais pas significativement associé e.Conclusion:L'é tude a trouvé une corré lation positive significative entre les taux d'acide urique sé rique et IR chez les hommes et les femmes et une corré lation positive mais non significative avec l'IMC, suggé rant que SUA pourrait pré dire l'activité des cellules bê ta des î lots.
