In conclusion, ALD remains a primary critical health concern, both in the United States and globally, due to its escalating prevalence and significant impact on liver-related morbidity and mortality. This review has underscored the dynamic shifts in the epidemiology and management of ALD, particularly in the context of LT. The increase in LT for ALD reflects not only the rising burden of the disease but also the evolving therapeutic approaches that aim to address the limited treatment options for end-stage liver disease. The integration of eLT for severe AH, and the increasing acknowledgment of its efficacy represent significant advances in the management of ALD. However, the challenges of posttransplant alcohol relapse and the necessity for robust support systems, including addiction counseling and comprehensive pre-LT and post-LT care, are critical for improving long-term outcomes. As we move forward, continuous research, patient-centered care approaches, and policy adaptations will be crucial in mitigating the burden of ALD and enhancing the quality of life for affected individuals.
