Background: Knee osteoarthritis (KOA) is a widespread, debilitating condition that drastically impacts the quality of life and healthcare systems globally. In Egypt, the burden of KOA is increasing due to an aging population, rising obesity rates, and lifestyle factors. Recognizing the urgent need for tailored recommendations, this article introduces the Egyptian Clinical Practice Guidelines for KOA diagnosis and management. These guidelines address Egypt's specific demographic, socioeconomic, and healthcare challenges. Results: The guidelines provide a comprehensive framework for diagnosing KOA, affirming clinical evaluation supported by imaging and laboratory findings when necessary. They integrate global evidence-based practices with local expertise and resources to ensure relevance and applicability. The recommendations prioritize a multidisciplinary approach, emphasizing patient-centered care through lifestyle modifications, physical therapy, pharmacological interventions, and the appropriate use of intra-articular therapies. For advanced cases unresponsive to conservative measures, surgical options are outlined. Special attention is given to enhancing the accessibility and affordability of treatments in alignment with the constraints of the Egyptian healthcare system. Conclusion: These guidelines aim to standardize KOA management, facilitate early detection, and improve patient outcomes across diverse healthcare settings. By offering clear, evidence-based recommendations, they serve as a critical tool for healthcare professionals, policymakers, and stakeholders, significantly reducing the burden of KOA in Egypt. © The Author(s) 2025.
