Results of LT for ALD have improved over the last decades, together with patient selection. While LT is the best treatment of patients with persistent liver insufficiency, some barriers still exist and access to transplantation center is still suboptimal. Early LT for AH has changed markedly the paradigm of the length of abstinence but data in long term are needed. A close partnership between transplantation teams and specialists in addiction will help manage patients with ALD after LT. Updates in alcohol relapse for early LT and long-term survival are mandatory to adapt guidelines and draw country-based and center-based policies.
