Endovascular repair of traumatic injuries of the subclavian artery

The aim of this article is to report the management of subclavian artery injuries using an endovascular approach. Traumatic injuries of the subclavian artery can be caused by penetrating or blunt trauma. These injuries can be handled with open surgical repair or using endovascular repair. Because of the location of the subclavian artery, many surgeons face difficulty when performing open surgical repairs. Besides that, this procedure still posed high mortality among patients. A 17-year-old male diagnosed with subclavian artery injuries was referred to our emergency unit. One day before admission, the patient complained that he was unable to move his arm. The patient had a history of motorcycle accident about 3 days ago and had been treated in the previous hospital. MSCT was ordered for this patient, and there was evidence of occlusion of the left subclavian artery. Endovascular repair was performed after the diagnosis. During the angiograms intraoperatively, we noted an occlusion left subclavian artery which later confirm the diagnosis of this patient. Following the repair, the normal vascular flow was achieved without any evidence of contrast extravasation. Due to the complexity of structure surrounding the subclavian arteries, open surgical approach become difficult to performed and posed a high mortality risk. Recent report has suggested using the endovascular repair as the preferable method for this condition. This approach offers the same effectiveness when compared to open surgical approach. Management of traumatic subclavian arteries injury can be performed safely and effectively using an endovascular approach which results in a reduction of operative times and mortality rates.
