Antibiotics have been well documented to result in several dermatological adverse reactions. Many of these adverse reactions are rashes that are difficult to distinguish. We present a case of a female patient in her 30s with acute generalised exanthematous pustulosis following vancomycin treatment for left breast cellulitis. Vancomycin was discontinued, and the patient was provided supportive care that included pain control, topical steroids and topical emollients. The rash improved over the following weeks. It is important to recognise drug reactions and differentiate between distinct dermatological conditions so that appropriate medical/supportive care can be initiated.
