Food protein-induced enterocolitis syndrome (FPIES) is a rare, non-IgE-mediated food allergy. It occurs most frequently in infants and young children, but adults can also be affected. Acute FPIES with severe vomiting 1 - 4 hours after eating and chronic FPIES with vomiting, diarrhea, and failure to thrive are distinguished and exemplified in two cases here. Pathophysiologically, a combination of immunological, microbial, and dysfunctional aspects is causative. The diagnosis is made by a detailed clinical history and can be confirmed by food challenges. Treatment consists of avoiding the triggering foods. The prognosis is good, and tolerance has usually developed by school age.
